I vote for miliary TB -- 3mo cough, Chinese, L apical cavitation with miliary lesions. Too many/too small lesions for septic emboli. Doesn;t look like inhaled process.
Miliary TB is named so because the nodules are the size of millet seeds (1-5mm with a mean of 2 mm). In addition, in miliary TB other organs are frequently involved (liver, spleen, kidneys, adrenals, meninges, especially in kids), because this is a hematogenous spread.
These nodules look larger, but they do look like hematogenous spead, may be it make sense to check other organs.
